Montgomery Medical Associates is excited to offer those patients being discharged home from sub acute rehabs or hospitals, Home-Based Primary Care services for follow up care, known as transitional care.  We also offer primary and acute health care to those with disabilities that make it difficult to see a provider in the office setting.

Home-Based Primary Care is designed to provide you with medical care in the comfort of your home and can be long term.

How is this different than Home Health?

Our Nurse Practitioner (NP) will come to your home and during your initial visit will review your medical history and complete a physical exam. The Nurse Practitioner will review and discuss your personal goals,  review medications, write prescriptions, order in home testing such as radiology tests  and blood work as needed.  

For those seen for transitional care, our team aims to prevent re-hospitalization or emergency room visits following your return home and to coordinate care with your primary care doctors. Your home Nurse Practitioner will visit you weekly for up to 4-12 weeks depending on your condition.

Home-Based Primary Care is designed to work seamlessly with your Home Health Agency. The Home Health Agency will provide you with Skilled Nursing, Physical, Occupational and Speech Therapy.  Our nurse practitioners can also help arrange these services in your home.

 

How are these services paid for?

Services are covered by Medicare, Blue Cross, United HealthCare, Aetna, Cigna and most commercial insurances.  Please contact our office to have your insurance verified for the in home services.
